REQUIRED FOR STUDENT PARTICIPATION

Forms 1 - 4 are required before students will be allowed to participate in laboratory activities. These can be handed in on the first day in the lab or emailed to NHAcadSci@gmail.com. If you've filled out these forms before and none of your information has changed, you do not need to submit them again.
